Application Form: The application form along with instructions is obtainable from the office of Haryana Public Service Commission, Bays o. 1-10, Block-B, Sector-4, Panchkula from 10.00 A.M to 1.00 P.M and 2.00 P.M to 4.00 P.M on all working days by making a payment of Rs.1000/- for general category candidate including DESM candidate. Rs.250/- for reserved categories i.e. SC/BC/ESM/PH (with at least 40% disability)/
and women of Haryana domicile only at the counter.
How To Apply: Candidate should ensure that the duly filled application form for admission to the examination must reach Commission’s office at Bays o. 1-10, Block-B, Sector-4, Panchkula by post or by personal delivery by close of office working on or before the closing date i.e. 31-05-2011 (07-06-2011 for applicants from forward remote areas).
