Application in the prescribed format. with 04 (One affixed on application form, 03 securely enclosed ina envelope) recent photographs with self signature on backside, attested copies of relevant certificates, self addressed reply envelope affixed with postage stamp of Rs. 5/- received at P.H.Q Itanagar and challan to the following address, PHQ, Itanagar on or before 28-12-2012 in a closed envelope superscribe on the top of the envelope in block letters with post name to be applied for.
